Request for patient portal access   Our patient portal, provides you with safe and secured access to your medical records. If you need copies of your records, labs, Rx, please bring this form to our front-desk, and you will receive your login information.

Billing & Insurance:

We recognize that medical bills can be confusing, and that you may want more information. We are available to help you and your family with any of your billing questions. There are a few things you can do before your visit to ensure accurate billing:

  1. When scheduling your appointment, per-register. Download Patient Information Form, you could fax it, e-mail it, or upload it to our website. This will ensure we have the correct information in our system.
  2. Make sure you have a good understanding of what is and is not covered by your insurance plan.
  3. If necessary, make sure your referring physician has obtained the referral required by your insurance company.
  4. Please bring all your insurance ID cards.

South Florida Nephrology Group, will file claims with your insurance company. If there is a balance due after payment is rendered by your insurance company, we expect your prompt payment of the balance. It is the patient responsibility to provide us with the current insurance plan information, prior to services rendered.  Please be prepared to pay your co-pay at sign-in. If you do not have insurance, we could setup a payment plan. Payment is excepted at the time of service, unless you have made prior arrangements. We accept: cash, debit cards, checks and all the major credit cards.
